jmn supplied me with files and enough info to find what the problems is. It is that the annotation code doesn't handle sideloaded kepubs properly. At the time the code was written, there were no sideloaded kepubs. Plus, Kobo has changed some things about how kepubs are handled.

As mentioned above, the bug for this was actually in the KoboTouch driver. The fix is in todays release of calibre. My testing shows it is working correctly, but I don't have that many annotations. If anyone sees a problem, please report it.
