When it comes to the premium catalog, errors are very often found later than the regular intake errors.
Well:
- You have a background-color and a background-image URL set in your page_styles css;
- You have an image (the signature) over this same background-image in the Foreword; thus, you are not only floating text atop an image, but floating an image atop an image,
- And you've repeated this error, using the background-image:url('../Images/kleur.jpg'); for the body class in the sgc-toc.css, as well.

Because they are
certainly there. That's what they mean by "overlays;" you're using a colored image block to emulate a background color (which, by the way, no offense, is just a dreadful idea; what about people who want to use Sepia mode, or night mode?). That's one of the reasons that they are rejecting your file. You also, as far as I know, can't use background colors in Smashwords, either--simple files are best, as far as they go.
I am unsure that Smashwords accepts SVG images for covers, FWIW. I don't really deal with Smashwords directly, but as a safety precaution, we make the ePUBs designated for SW with JPEG covers. Also, premium-catalog-wise, don't you need a minimum 1400px in width for the iBooks store, for the cover? I believe so.
