calibre defaults to leaving the book as-is in the library, but will update all supported metadata in a book when sending to device or saving to disk.
Cover, btw, is only supported in EPUB/AZW3.
Polish Book will force calibre to update all that metadata on the copy inside the library as well. It only works on EPUB/AZW3, and will not even update title and stuff for .mobi and others.
Polish Book does not (by design) utilize plugboards.
Polish Book should only be necessary if you use Dropbox/calibre2opds/some other method of accessing the calibre library files directly. Or if you are using it's other functionality, of embedding a book jacket etc.
