Press the up and down arrow multiple times. The first press will have the same effect as the left/right arrow and the second press will have move the item up/down
And existing entries are deliberately not replaced. That allows you to add to an existing toc using the generation buttons. If you want to replace, delete first then replace. Use Ctrl+A to select all and click the delete button.
