Unless something has changed recently, when you install a Calibre update one of the windows along the way has a "Custom" button option which when opened allows one to deny installation of a desktop shortcut (it may not use those words). In my experience the tile on the Windows Start page is not changed in any way and I assume that should you use a desktop shortcut (I don't, I have the Start Page set up to be far more efficient for my own purposes) if you deny adding a short cut in the Calibre custom install it will leave any existing one alone.
Note, I have not tested this as I have 1.31 installed already so can only test by uninstalling and reinstalling (with it installed already one gets the repair, change, etc options type window).
