Alternatively, on FW 5.3/5.4, there's a known [to us, at least ;P] (upstream) issue, where the updater watchdog sometimes badly fails to do its job (and then potentially crashes). Clear the USB root of .bin files, restart your device and try again.

The KUAL button is useful if you push the .bin file through any other means than USBMS (usually, SSH). It doesn't help with the issue previously mentioned, though.
