The link is not properly formed for a URL and thinks it points to a local (missing) file in text Text folder.
Wrap the SVG in a <div>. If you had use Tools: Add cover (it will replace the existing file.
Caution: The file marked as Cover should only be a cover or the rest of the file will be lost)), this would have been included
You changed the file after the TOC was created. Regenerate the TOC or carefully edit the OPF
