You should be able to do both Remove existing cover and Insert or replace cover in the same Modify run.

To 'make sense of it' we would really need to know what options you used, and the easiest way for you to do that is to post an screen shot of the Modify settings - if you don't know how to do that, then tell us what OS your using and someone should be able with help you with that.

Nor do I understand why Polish and Modify behave differently. But my experience is that the behaviour of code is determined by its content, the context in which it's used (Polish v Modify), and by the platform on which it's executing (Linux v Windows, your computer v mine etc).

Yes you could check with kiwidude, but if the existing image at the front of the book is not designated as being the cover, then Modify can't be expected to remove or replace it. Inside an EPUB the image to be used as the cover is designated within a section called the titlepage.

If you want to take a look at it then let us know and someone will tell you how do it, its not that its hard or a secret but... information overload...
