Calibre Companion (CC) does not extract the cover from the book, CC always uses the cover that is showing in the calibre GUI.

Possibly but as I stated above if the cover is not properly identified then Send to device may not be able to update the cover.

Certainly worth asking. Also the app may cache covers and the cache may have to be deleted along with deleting and resending the book.
I personally don't see the appeal since I use CC to find and launch my books I don't care if the reading app shows a cover on their bookshelf.

It's never safe to assume anything. CC doesn't rely on the book to get the covers.
