The book needs to be one bought from Kobo. These get downloaded directly to the device using WiFi or through the desktop application. I have had a couple of problems with these books when loaded from the desktop application after a factory reset. Removing them and downloading them via WiFi fixed this. The problems I saw were not what you are seeing, but I would like to check.

And I had a look at the book on both my Touch and Glo. Neither had a problem with it. I also had a look inside. The code was pretty clean. There were a couple of warnings for the CSS, but they didn't cause any problems. I cannot see any reason for this to cause a problem.
