Yes, the problem seems to be when the book doesn't have the same title (at least that's where I've noticed it) which can happen when the publisher updates it, or through calibre, or Java Mobi Metadata Editor, etc.

And it doesn't help to remove and re-add the new copy, you must remove it using the old copy! What is up with this I do not know....
