I think it is safe to say Calibre companion has nothing to do with your troubles. But if it is, then you need to be more specific and present the problem as if the person your explaining it to has never used fanficdownloader.

Is the story a single updated epub?
If it is a single epub are you deleting it from Calibre companion before sending the new epub?
Do the epubs have the same name?
Do the updated epubs use the same book entry in Calibre?
Or are the chapters separate epubs?
The fact you have used both the Content Server and direct transfer via wireless would indicate that the issue is most likely in your work flow.

If the book being sent has the same name as the book on the device then make sure to delete the book via Calibre companion on your device before sending the new book. Double check the exact contents of the reading list.
I only have questions and guesses hopefully someone with experience will shed some light on our problem
