Edit: Kovid already stated this in post 7. I should pay closer attention. None the less here is the same info layed out in a different fashion, I hope it helps.
The book covers for these books never showed in any previous version. When you place a book in the library and adjust the metadata including cover for a book, that new cover is not automatically embedded in the book (epub or azw3) until the book is converted or exported from calibre using most any existing calibre feature.
- Send to device
- Save to disk
- Email from calibre
- D/L the book from the content server
Additionally the following features will embed the cover and metadata in a book that is still in the library:
- Converting the book will embed the cover and metadata in the converted book.
- Polish feature will embed the cover and metadata in existing Epub or Azw3 books.
In your first example (post 1) the default cover exists in the book because there was no existing cover in calibre when the book was converted to that format. Ever since the conversion with 1.15 that generic cover has been embedded in that book.

You can embed covers in books within your library by following Kovid's advice. theducks has already told you where to find the tool so you can add it to a toolbar or menu.

Going back to any previous version will not help. The problem was not caused by the upgrade, you only noticed the issue after you upgraded to 1.20. For the books that exhibit this error they have always exhibited this error.
