What works well for some PDF's won't work well for other PDF's. Some just don't convert well at all. As willus said it really was designed and an "end" format and not to be coverted back to other things. What I usually do it use Acrobat Pro to save as HTML (then create an ePub and convert that for Kindle) and then go from there. For basic text based PDF's this usually seems to work fairly well and require minimal work after the conversion.
