Cover art is up to the publisher. Some publishers (most these days) include it, some don't.

The number of pages in ebooks is counted differently, depending on the software. I don't know about Kobo's on-device software, but Adobe Digital Editions page count is often much, much smaller than the hardback or paperback versions (I assume, among other reasons, that ADE leaves out all the chapter start and end half-pages, empty pages, etc, when counting the pages, in addition to other things).

Your chosen font size, margins, line spacing shouldn't have any relevance to whatever page count is given - it's likely that you'll be on the "same page" (same page number will remain) for a number of page turns.
