I've gone ahead and sent Kobo support an email (with screenshots showing the table of contents of the Kobo-bought edition in Adobe Digital Editions and an Amazon Look Inside screenshot of table of contents for comparison). Can't hurt, I suppose.

I've also started Un Lun Dun by China Mieville, but haven't got very far yet.
