Are you by chance using iTunes on a PC? If so I have had a problem in the past that if I re-name a file or change its location iTunes will "fix" it for me and put it back to what it was! The only way around this for me was to remove the file from iTunes, rename/move it, then add it back.
Of course, why would I ever do that? Apple knew best that's why they fixed my mistake...
