AZW3 (but not MOBI) recognizes the cover as supported metadata, and calibre will fix this. Just use calibre to set a new cover (Edit Metadata ==> download cover) and then Send to Device.
Do NOT do an azw3 to azw3 conversion! This performs MANY changes in addition to updating the cover, that is why the book is no longer the same book!

I tried this and lost the X-Ray feature, should I erase the book in the Kindle itself before resending it via calibre?
Edit: Disregard the above, sorry, it seems like the X-Ray feature just takes a few seconds to kick in and stop being greyed-out.
I erased all versions of the book from my Calibre Library, connected my Kindle, copied over to my Library the original AZW3 straight from the cloud. Updated the metadata, changed the cover, and used "Send to Device".
The Kindle itself shows only one version of the item, but browsing the device in Calibre I see TWO versions, one that Calibre shows the cover preview for and one that only shows the default icon where the Preview should be .... and it seems like no matter what I do the cover isn't changed: "Go To > Cover" takes me to the title page, like it does in the original file. Any ideas?
