If you add the book to calibre, and update the metadata, then resending the book to your kindle from calibre will update all supported metadata on the fly. It does this with minimal changes, by only overwriting the changed fields, and it will be recognized as the same book.
AZW3 (but not MOBI) recognizes the cover as supported metadata, and calibre will fix this. Just use calibre to set a new cover (Edit Metadata ==> download cover) and then Send to Device.
Do NOT do an azw3 to azw3 conversion! This performs MANY changes in addition to updating the cover, that is why the book is no longer the same book!
