My kindle is the PW2 Wifi only International edition (B0D4). I'm running the latest firmware (5.4.2) and I have tried to uninstall and install simple usb several times.

Most frequent problem:
The cable has two ends, and two IP addresses, one for each end.
What IP address is set (in the Kindle) USBnetwork config for the Kindle end of the cable?
What IP address is set (in your PC) config for the PC end of the cable?
What IP address are you trying to access when you get "connection refused"?
