Title Page Issue - ePub to ePub conversion

I'm having an issue where the title page in some of the books I've converted has disappeared from the ePub (ePub to ePub conversion). These are title pages where there is no text only a photo. I do not have the option to "Remove first image" checked on the Structure Detection page in conversion options.

The format in the original ePub is

where XXX is the book title. They all have img alt="[image]" whereas with other books I've converted where the title page stays the coding is different.

I'm sure there's some obvious reason the whole page disappears that I'm not seeing. I've tried converting to LIT and then back to ePub but the title page disappears still.

This is a small number of books, but I would like to understand why it is happening. Thanks.
