I don't know how YOU added the imag (I guess you tried directly).

In Sigil it is 2 steps (to get it to show. more to style)
1) Add Existing Files <the image>. You should now see it in the book browser listed in Images if you did this correct.
2)Place a (the) image using the Black screen Icon
