The majority of time my books come to my kindle with about the same size so that size 4 looks about the same in most all of them. The spacing is different sometimes, but not the size itself. Once in a while I get a book that has some weird formatting and I have to fiddle, but rarely now.
Most of my books come straight from the amazon store to my kindle though as that is where I buy pretty much all of my books.
So I don't have that variation of size.
