I'm new to this, but I've tried several different conversion methods and I *always* get errors related to the Table of Contents, specifically:

So far in Calibre I've tried:
- .html to .epub
- .docx to .epub, with functioning docx Table of Contents
- .docx to .epub, with Table of Contents inserted by hand in Calibre
I've already tried tweaking Calibre's interpretation of the ToC, and in addition to creating my own ToC in Calibre, Calibre itself verifies the ToC layout.
What gives?
