I don't believe it's possible to provide a persistent link in the manner you suggest since charity selection is initiated by the customer and tied to his/her account.

Note that the charitable organization needs to register with AmazonSmile in order to receive donations allocated to them. There's more information about that here (AmazonSmile Org Central). You can find contact information there, too.
