Importing these into Calibre (from the local file system) is then going to cause issues if retrieving that internal title instead.
So the point is more a how to get them into Calibre in the first place without the first imported copy getting overwritten or the second added copy ignored depending on merge settings, when you want both or however many copies you have

The OP wants to display the original file name on the Kobo reader. Getting the file name in is easy. All they have to do is set the import template to put the full file name in the title. Then it goes into the title metadata when the epub is sent to the device and the Kobo reader will pull this out and display it.
But, depending on why the OP wants to do this, there might be a better way. I think that populating calibre with the correct metadata works better in the long run. If there is a specific display needed on the device, then a plugboard will usually be able to do this.
My guess from what the OP said, is that the file names contain series info or other reading order information and wants that replicated to the device to make it easy to work out which book to read next. If I am right, then a plugboard is ideal. Or, if it is series order, the Kobo devices have support for display series info. Using the Kobo Touch driver and my Kobo Utilities plugin, the books can be put into shelves in the correct reading order.
Whenever I see a question like this, I have to ask what they are really trying to achieve. Once you know that, maybe you can come up with a better solution. As they say, there is more than one way to skin a cat. Knowing why you want the cat skinned can help to choose the best method.
