I've done a number of epubs for books not available otherwise (mostly with dead writer and no edition otherwise available, electronic or deadtree). My editions are definitely better than the average retail, dropcaps (where applicable) and all. It's true that I don't use the typical retailer filenames (e.g. cvi) but I could if I wanted or if I were using some retail publication's opf and ncx files as templates.
