Okay, this time I do not know for sure what the exact problem is, but try this:

On the page setup tab, make sure output profile is set to Kindle Paperwhite.

You'll have to convert from the original file again to see if that worked. If it doesn't, I'm really not sure what's happening

That's the only other option that seems meaningful to the issue that I know to change in Calibre.
