If you go into edit metadata, you can set the cover image for the book from the PDF file. In the edit metadata screen, click on the PDF format of the book, so it is highlighted. Then click on the brown book icon. This is to 'set the cover of a book from the selected format' option. It will display several pages from the PDF file, choose your image, click ok/save, and you should be good to go.
