I have a bunch of PDF with the proper cover in it but it doesn't show it as the book thumbnail in calibre.
It seems that the only way to see a cover is to have the cover.jpg within the folder itself,
I am missing something here ?

First never place/change/delete content directly into calibre folders. Calibre logs and adjust things when it 'shelve' things in the library.
Changes should only be done by using Calibre GUI or CLI. (there are certain sleight-o-hands changes that can be done by knowledgeable persons or plugins)
cover.jpg is the name Calibre
assigns to the cover image calibre uses 1)in the Details view (and stores it in the books designated folder). 2)Send to Device/Save to disk. There are others (thumbnails) that are cached, but transient and will be rebuilt from the Library image.
The cover (as defined INSIDE the book) may be a different image, a different filename. (in EPUB the internal OPF designates the filename used)
