Polish books is largely a front-end to parts of the conversion process. And my memory tells me that Modify ePubs calls the cover handling parts of the conversion. Both will handle adding or replacing the cover in the same way a conversion does. That includes using your default conversion settings.

So yes, they will add the cover to the file, or replace the existing one. They attempt to change the existing cover page, but if it can't find one, a new page is added at the front for the cover.
