With Koreader the covers are on the first page, if they are embedded in the file. What I do for epubs without embedded covers is run them through calibre with the metadata set as I wish including the cover I want and then do an epub>epub conversion. That gives me an "original-epub" and an epub with the cover embedded, in the calibre library. I copy that epub to my device.
