As an explanation of what happened:
The firmware caches cover images for performance. That way it doesn't have to reread them from the drive. If you replace the cover image, something has to trigger a reread of the file or it is no longer being used and is dropped from the cache. For the old style home page, the main cover was dropped from the cache when the book stopped being on the home page. With the new home page and firmware, this isn't the case. Doing a restart will force the cache to be rebuilt.
But, deleting a book and the image not being removed from the cache is a bug. Hopefully Kobo will fix it, but I can see that it would be a low priority.
