That confused me too. I initially assumed you would choose the second option which says you've exchanged it, but it seems not everyone has taken it that way so I'm not sure. I think 3 options would have made things more clear.
I'm satisifed and I kept it
I exchanged it and I'm satisfied with the replacement
I wasn't satisfied and returned it for a refund
