How did you add the cover image to the EPUB (Tools - Add Cover?)?
What is the code on your cover page?
What is the resolution of the cover image?
Does Calibre cut off the thumbnail as well?
Does ADE cut off the thumbnail as well?
Does this happen with all your EPUBs or just this specific one?
