IIRC amazon's azw3 does have some information in the ebook to relate reading position to the page numbers in some dead-tree edition. This information will probably get lost, but as I don't use azw3 (or mobi) I don't know that for a fact.

You can tell CC to put books wherever you want. As DoctorOhh said, the process is explained in the faq. See the four questions and answers at Folders and file names (the sixth dot).
