I haven't tried TTS for a couple of years on Windows, but the last time I did, the quality of the voice output was not very good. Slightly better than Stephen Hawking's infamous voice output, but not substantially so. Additional voices were becoming available, and were far less "choppy", but only as paid enhancements. Perhaps things have changed since then for Windows.

If you're using some other reading device or operating system, there might be other options.

For instance I believe any Android 4.x (Jelly Bean) device would allow you to use Moon+ Reader which offers TTS. I've got the Pro (USD $5) version, not sure if it is available in the free version. This program is only available for Android, no other OSes are supported.

I would imagine that Apple offers some kind of TTS functionality also, but I'm not aware of anything specifically (free or paid).
