Alt Text in epub

How important is it for accessibility when InDesign exports images based on default file names? I really hate the whole concept of specs and in practice it's a big waste of time because no ereader I'm aware off that allows users to simply listen to alt text by turning off load images/video/audio...

Realistically, how many actually spend the time to add proper meaningful alt tag into epubs?
