I sent an email to the address listed on the settlement website regarding my Sony purchases and actually got a response an hour later! In part it said:

You should look for a notice over the next couple of days. You have already filed a check request for these settlements last October and do not have to submit another one.

That's where I had the most eligible purchases with ten titles; only two each at B&N and Amazon so at least I know I've got
something coming to me.
