That sounds like a problem that has been discussed with kepubs. The circumstance seem to be a kepub with embedded fonts, using "Document Defaults" as the font and then using the TOC to get to a chapter. From memory, it also happens if you use a bookmark or reopen the book. Try changing to another font and seeing what happens.
