Create a simple TOC as outlined in this FAQ. Then place the TOC.html and the other three html files in the same directory (any temp location outside of the calibre library will do.) Now drag the TOC.html to add it to calibre. Calibre will add all 4 files to a zip file and add it to calibre as one book. You can now update the metadata and convert that book to the format of your choice.
