I'm not sure I understand what you are asking, but I'll take a stab at this.

If you buy the books through a Kindle, they will download automatically since you are will have had to establish a 3G or WiFi connection to get on Amazon's site. However, you can buy books through another device that has the Kindle app on it, such as an iPhone, or you can get onto Amazon's site through a browser on a computer, and log into your own account. Then, when you buy the book, you can choose which device to send it to. So, you can buy the book on a computer, for example, but choose to send it to your Kindle. The book will not download though until you connect the Kindle to 3G or WiFi.
It's not unusual for me to have 10 or more books (freebies, I swear

) that have been purchased but not downloaded. They just stay in the cloud until I connect whatever device I have chosen to send them to.
As for Canada vs. US, I travel extensively overseas (my homebase is the US), but it doesn't matter where I am, I'm buying books from my home US account. If I turn on my Kindle in Europe and buy a book, it is charged to my US account, and I do NOT get charged the document delivery fee that is standard for accounts not in the US (I don't know about Canadian accounts), nor do I get charged VAT or anything else. Just last month, I was in Indonesia, used both my 3G and WiFi there, and bought several books.
