I have had a lot of success using InDesign. No need to use Calibre, or ePub, or KindleUnpack, or make any changes to the ouput at all.

I used Amazon's free InDesign plugin - it MUST be the latest version. I've not had any problems with the latest version. I was not using Adobe's ePub output.
