I understand Hollywood has similar problems. There are people whose whole job is to make sure things are as consistent as possible. For example if the hero is smoking and he's just lit a cigarette (far shot) it's their job to make sure that the cigarette isn't 3/4ths burned away (in the close up) a few seconds later. Of course errors do still crop up. The old blind man with the golden idol in "The Ten Commandments" is a classic screw up. He's wearing a Timex wristwatch!
