You need to do more work at the HOST end.
CC (and almost everything else today) works on a LAN.
Your Router/firewall separates your LAN from the WAN for safety and security (and because your ISP only allows 1 IP on a connection)

You need to have done:
Assign/reserve a fixed (LAN) IP for the host

Set 'Port Forwarding' (allows a WANside port request to a preset host-server) to the designated IP

Setup a method for obtaining the current WAN IP of your router. (DynDNS type service, ISP provided Static IP, A neighbor with access to your router/WAN that can query it and tell you what it is right then
