I believe their is an "include cover in ebook" option in Preferences or Tweaks that will update the book when a new cover is set. Otherwise this occurs when converting a book. Of course, there is a catch - I know at least one of the ebook formats doesn't accept cover images.

Someone else just posted a similar question very recently (week or so ago).
I'm must be missing it...i cannot find that option
