I use a polishing plugin for Calibre that polishes up some of the issues with an Epub I wonder if it might sort the issue out you are having, you would have to remove the book from your ereader and then restart the ereader and then push the book once again to your reader before any changes would occur on the reader. (well for me that's how I have fixed some issues, and maybe you wont have to delete and restart etc just how I do it I guess.)
