I did some more digging, and the issue is slightly different than I first indicated. Whenever a new page (usually a chapter, but not always) is loaded, the text starts halfway down the page, no matter what the css says. I am currently reading a book that uses a top margin of 1em for the title (and for things like the copyright page). Instead, every new chapter starts halfway down the page. Subtitles and other text within each chapter display just fine, and respect the css margins.
This is with side-loaded epubs.
