Correct.

As a general rule, all changes done to library entries in calibre are initially private to calibre. This is because calibre relies on its database to provide the information rather than re-reading it in each file every time you display a library. Re-writing and re-reading this data in the actual book files would take a prohibitive amount of time with anything other than a trivially small library.

This is also true of the cover image. Calibre extracts a copy for its own use and changes aren't re-witten into the original book file unless you take certain explicit steps.

Most ebook formats require that the book file be rebuilt to make these changes, a relatively slow process. A very, very few formats allow for simpler "insertion" of such changes, though many things can be done in two of the currently most popular formats, ePub and the newer Kindle "KF8" (aka "AZW3") formats. Still, special steps need to be taken to insert changes into these formats.
