As explained by HarryT downloading a cover does NOT update the ebook (format) files themselves, all it does is to put the image as cover.jpg into the book folder. Then the OP was given some methods to update the ebook file(s) - including Polish (shortcut P), along with instructions on how to put Polish in the icon bar.

Its not clear from the OP's post how the book was loaded to the IPad, or whether it was reloaded after the cover was downloaded - if it was done via a OS level file copy then the old cover would still be there.
